緊張型頭痛は一次性頭痛の中で最も多い頭痛の一つである．このなかで頻発型緊張型頭痛と慢性緊張型頭痛は支障度の高い頭痛であり，従来から急性期発作時療法と予防療法が用いられてきた．ここでは，その治療の必要性と選択肢についてエビデンスに基づいて解説する．

ICHD- ㈼分類 ¹⁾ では，反復性緊張型頭痛を，頭痛の頻度が月あたり 1 回未満の稀発型と頻発型にさらに細分類することに決められた．稀発型が個人に及ぼす影響はごくわずかなものであるため，医療関係者からあまり大きくは注目されていない．市販薬が効かない場合や特別重症な頭痛発作でない限り，医療機関による治療の必要性は少ないと考えられる．しかし，頻繁に罹患することにより，時として高価な薬剤や予防治療薬が必要となることがある．さらに慢性型に分類される頭痛は生活の質（ QOL ）を大きく低下させ，高度の障害を惹き起こす深刻な疾患であり，治療は必要となることは個人的，社会的見地からも当然のこととなり，経済的費用負担を伴う．
　慢性緊張型頭痛においては中枢性疼痛メカニズムがより重要な役割を果たしているのに対し，稀発反復性緊張型頭痛については末梢性疼痛メカニズムが役割を果たしている可能性が最も高い．したがって，頻発反復性緊張型頭痛と慢性緊張型頭痛では治療が必要であると考えられる．
緊張型頭痛の 治療法には急性期（頓挫）療法と慢性期（予防）療法 があるが，それぞれに薬物療法と非薬物療法がある．急性期治療，予防治療の別に関しても，片頭痛とは異なり厳密な区別は不可能である．しかし，予防的に薬剤が投与される場合は副作用を防ぐため 3 ヶ月以内の投与が目安となる．
　 さらに，緊張型頭痛を引き起こす因子である心理社会的ストレス，不安，うつ，運動不足・うつむき姿勢そして口・顎部の機能異常に対しての個々の基本的な治療法も重要である．
